New and seeking advice

I've been reading through for a while, and am inspired by many of your successes, and feel others pain.
I am 36, white, male, 6'3, 360 lbs.
I have a very active job that keeps me on my feet for 12 hours a day, and usually 70-80+ hour work weeks. I also travel constantly and have spent a good deal of my adult life living out of hotels.
I was always the chubby kid. I've struggled with weight loss since I hit puberty. I slimmed down for a few years when I lived in England, and walked everywhere I went, plus couldn't afford to eat much more than canned beans on toast. But back in the states, and the weight came back on.
Ive tried multiple weight loss programs and diet aids. Everything from fin phen back in the day, to HCG, to weight watchers, to the juice diet. I've hired personal trainers, but with my work schedule, it was difficult to meet regularly. I eat a fraction of what my friends and co-workers eat, but they are all skinny, and I'm still huge.
Right now I'm working in Albuquerque. I start the day with a green shake using my nutriblast. I try and have a salad, or something small in portion size for lunch along with a protein shake, and for dinner I'll get a wrap or something like that. If I feel the urge for something sweet, I make a jello sugar and fat free pudding. I've also been trying Nutrishops Adicore, which isn't as good as they claim, and is very expensive, but it does help.
Doing this I went down from 385 last year, to 350 by Christmas. I was very good over Christmas and stuck to my regiment. In the last month however, I've hit the wall and jumped back up to 360.
One of the many downsides to traveling all the time is every time I move, and I'm usually in a place around 6 months at a time, I have to find a new doctor. Often I don't bother, because they always tell me the same thing, loose the weight, but don't offer much helpful advice. If anyone knows a good doctor in Albuquerque, I'll be here till October at least, and would love a reference.
When I visited home over Christmas, and saw the family doctor, who is very old school and only believes in exercise and hard work, he told me that if I don't lose the weight soon, I'm going to have serious health complications in the near future, and he recommended looking into weight loss surgery.
I don't like the idea of weight loss surgery. First off, my brother had it, the Lap Band. It was effective for the first month, but then there were complications. They released the band to sort out the complications, and it shifted position to where they have to go in surgically and reset it. He paid for the first surgery out of pocket, and can't afford to pay again to have it repositioned.
Plus I just feel like the human body should be able to take care of itself, and I am not a fan of anything surgical. I had cancer as a kid, and have seen my fair share of hospital rooms and surgeries. Plus my insurance won't cover it either, so it would be out of pocket. If I'm paying for it anyways, I've started looking into other options.
I've started looking into fat camps, or weight - loss retreats. I feel that with an externally monitored diet, and exercise routine. I figure for the price of the weight loss surgery, I can spend a few months at a weight loss retreat, and hopefully see some real improvements. I know there is the risk of falling into old and bad habits when I get out of the carefully controlled environment, but I still feel it is a better option than going under the knife. It's either that, or hire a live-in 24/7 personal trainer for a few months. Take the time off work, hit it hard, basically make him my jailer. The plus side of that would be more one on one, and good habits would be formed while in my home, therefore easier to maintain after. The only issue there is if we don't get along really well, it could be a very long couple of months.
Has anyone here had any experience with fitness camps or live in trainers? I know I've worked it all out in my head, but am very interested in some external feedback. Also, if anyone here has tried any of the fitness camps, I would love to hear about your experience. Especially how long you were there, and how much you lost, and whether or not you would do it again. I'm not sure I trust their webpage success stories, just as I don't trust anyone who tells me they lost 70 lbs on a certain pill, and come to their website and they will sell it to me.
